Diabetes Education Programs
Diabetes education programs offer free classes and individual appointments for people with diabetes and their families. The education is offered by certified diabetes educators, who may be registered nurses, dietitians, social workers, pharmacists or other health care professionals specializing in diabetes. Diabetes education programs are excellent learning environments that offer education and support to properly manage your diabetes. To attend a program, ask your physician, nurse practitioner or other health care professional to send a referral or you can refer yourself using a self-referral form. Central Intake© will direct your referral to the most appropriate program for you, and the diabetes education program will contact you for an appointment.
