Sickkids Hospital Research Trial - Transition to Adult Care
If your practice involves caring for patients with T1D, please consider taking part in this worthwhile research study. SickKids would like to speak with people living with diabetes between the ages of 18-24 years, their caregivers, the Healthcare Professionals (HCP) involved in their care as well as Healthcare Administers. The goal is to gain a better understanding of each of these groups' experiences with transitioning from pediatric diabetes care to adult diabetes care and establish a set of key quality indicators for a good transition experience while ensuring high-quality care.
Please consider posting and sharing the research study flyers in your clinic or office.
